Extended Services
Information and referrals to social service entitlements are key to providing seniors and caregivers the tools necessary to ensure the optimization of resources available to them. Our comprehensive programming allows individuals to obtain access to these benefits as well as the full range of available services through HANAC's multiple programs. The program is a valuable resource to members of the community that they may utilize to stay up-to-date on entitlements and benefits as well as obtain assistance towards applying for benefits or to receive any necessary referrals.

The Extended Services Program provides case assistance, information and referrals related to:
Social Security benefits information
Medicare, Medicaid
SNAP (Food Stamps)
Public Assistance referrals
Reduced Fair Metro Card
Translation Services
Heating Education and Assistance Program (HEAP)
Elderly Pharmaceutical Insurance Program (EPIC)
Senior Citizen Rent Increase Exemption (SCRIE)
Disability Rent Increase Exemption (DRIE)

HANAC NWQH (Northwest Queens Housing)
The program provides information and case assistance in regard to housing related issues, as well as information on affordable housing and assistance with applying for housing.

Over 120 individuals per month are assisted with concerns related to:
    Landlord/Tenants problems
    Housing counseling
    Housing applications for affordable units
    Lease assistance
    Affordable housing application assistance through the NYC Housing Connect website
    NYCHA (New York City Housing Authority) applications and renewals
    Section-8 recertification assistance
    NYC-210 and IT-214 Tax Refund applications
    Home Energy Assistance Program (HEAP)
    Cooling Assistance Program offers a no-cost air conditioning unit for qualified applicants
    Lifelife Discount Program that offers a discount or a no-cost cellphone for qualified applicants
    Weatherization services referrals
    Senior Repair Program that offers minor home repairs
    HANAC Caregivers Program – SNAP
    The program provides caregivers who are responsible for the care of a senior or a disabled person with information and referral services, counseling and case assistance to better serve their care receiver. The program aims to provide support for caregivers that are caring for a parent, spouse, friend or other person with illnesses such as Alzheimer's, dementia, and other chronic illnesses. In addition to case assistance with entitlements and benefits, the program assists caregivers with available telephone-based or in-person counseling services, respite services, supplemental reimbursement and educational workshops.

    A caregiver can be anyone who:
    Seniors taking care of their spouses
    Children or grandchildren taking care of their parents or grandparent
    Neighbors or friends taking care of older adults living down the street
    Grandparents taking care of grandchildren

    Housing Ambassador Program
    HPD has partnered with HANAC to provide community-based service assistance for people to prepare and apply for affordable housing. As certified Housing Ambassadors, HANAC Extended Services staff members help provide up-to-date, consistent information and assistance about the application process to communities, and we encourage people to contact us for guidance.

    HANAC/HPD will host meetings with community members in order to inform them about the application process for affordable housing.
    NYC Housing
    Housing Ambassadors are community-based organizations and service providers who partner with HPD to ensure that people have access to up-to-date information and assistance with affordable housing applications.

    Due to the COVID-19 pandemic, all case assistance, information and referral services are being conducted remotely over the phone. For the time being, we are not accepting in-person appointments. Please contact our office for further information or to schedule an appointment.
